Investigator Database:
- Enter your personal information (Enter Personal
Information)
- Download and print the Application Form (Download
Application Form)
- Sign the Application Form
- Attach your IRB Approval or Exemption to the Application Form
- Send the application and IRB Approval/Exemption forms by mail

- After your application has been approved, you will receive your
Password via mail. Using the Username you have selected and the
Password you will be assigned, you can then access the TPF Repository.
